Matchmaking lock-out?

I keep joining matchmaking games and seeing a green square for myself, and for most of the other players. 1 or 2 players usually stay dark.
Then when the game fails to start, I get the dialog saying that I caused the game not to start, and after 2 of those I get the 10min timeout.
What does the green square for a player in the matchmaking setup matrix mean?
What do the yellow squares mean?I had assumed that the players with green squares weren't the ones causing the lack of connections, but I must be wrong, since one can have a green square and still get a timeout.

Green square means there is a connection between those people. Yellow square means there is not a connection between those people. Game can't launch unless all squares are green.

If that's what the colors mean, then what is the meaning of:
- The square representing me, starting as yellow, and later becoming green. It doesn't flip to green only when all the other squares are green...
- One of the other squares going from black to yellow. If green means "I'm connected to that person", then what is the difference between black and yellow for that person?

I'm not sure if the server that is responsible for the penalty knows who caused the game to fail to launch.

The game is peer-to-peer. Everyone is connected to everyone else. That's why there's a matrix. Usually you only see your own connections. What you see there is a best-effort to sync the entire network state.
