To replicate: right-click with your T2+ engi on a structure under construction, then quickly SHIFT-right-click on a T3 mex, and fabs around that mex will be ordered, even though you are supposed to SHIFT-double-right-click on a T3 mex to surround it with fabs.
I would provide a replay, but I doubt there is a way to see if I double-clicked
