Help me understand connection window before match starts

Can someone please help me understand what the different color boxes mean on the connection window? I always took a green box as meaning I am connected, yet it happens often where I am blamed for a failure to connect and then banned for an x amount of time. Though once a game manages to launch it runs fine. I can also play other games online without any issue so it's not my internet that's the trouble.

FAF is p2p based (player to player direct connection, no dedicated server), it's just a matrix telling you who you are connected to.

If the box is green: connected to you and everyone else
If the box is yellow: connected to you but not everyone else
If the box is black or non-existent: not connected to you -
But I will have green box and still be penalized when the game fails to launch
